Patient Forms

New patient registration forms are located below and can be filled out prior to your initial appointment. Along with the New Patient Registration paperwork, new patients to the office are to bring in their driver's license/photo ID, insurance cards, and a list of medications they take including the name, strength, and how often they are taken.


You are responsible for knowing if your insurance requires a referral. If so, you are also responsible for contacting your primary care physician to obtain that referral.


Please read over our instructions to see how to properly complete the new patient paperwork. If you have any questions, please give us a call at any of our locations.

 For patients being seen for sleep evaluation, please print and fill out an Epworth Sleepiness Scale evaluation found below prior to your visit and bring it in along with your New Patient Registration forms.

For patients seeing our allergist, in addition to the New Patient Registration forms above, please fill out the allergy history assessment form found below for questions relating specifically to allergy.


If you are having allergy testing done, you will need to stop any antihistamines 72 hours prior to testing. For a list of common oral antihistamines or for more information, see our allergy & immunology services page here.
